Handbook Receipt and Acknowledgement

I acknowledge that I have received a copy of the Barber DME Supply Group, LLC Employee Handbook. I understand that it is my responsibility to read and comply with the policies contained within this handbook, as well as any revisions made by BDME in the future. Failure to comply with the Employee Handbook, may result in disciplinary action up to and including termination of employment.

This handbook describes important information about BDME, and I understand that I should consult with my manager regarding any questions not answered in the handbook. I acknowledge that my relationship with BDME is at-will. Accordingly, either I or BDME may terminate the relationship at any time, with or without cause and with or without notice, provided there is no violation of applicable federal or state law or breach of a signed contract.

I further understand and agree that no employee of BDME, other than the Managing Member, has any authority to enter into an agreement for employment for any specified period of time or to make any agreement contrary to the above. Any such agreement must be in writing and signed by the Managing Member.

I understand that this handbook and the policies contained within supersede any prior policies, oral or written, regarding the terms and conditions of my work with BDME. By distributing this handbook, BDME expressly revokes any previous policies or procedures inconsistent with the policies contained herein. I acknowledge that BDME reserves the right to revise, supplement, or rescind policies, practices, or benefits described in this handbook at any time, as it deems appropriate.
